If you’ve been referred to our practice by your primary care physician for rheumatology treatment, here are a few things you can do to make your visit with us easier and more efficient.
- Complete your New Patient and Health Questionnaire forms. (These will be sent to you before your visit.)
- Make note of any recent lab tests or other procedures you’ve had on the recommendation of your referring physician — we’ll want to be sure to obtain the results of these and avoid any unnecessary duplication.
- If you’ve already been diagnosed, we’ll need to know your diagnosis. Please bring along all information about your past and current symptoms and treatment. It’s also good idea to write down any questions you have for the doctor, and any new symptoms or other information you’ll want to be sure to remember during your appointment.
- Contact your insurance company before your visit, to be sure that there’s no paperwork required to secure coverage for your fees.
- Bring along your insurance/health system card(s) and ID, and, if you’ve been seen by us before, please be sure to let us know if your insurance plan/carrier changes so we can update our billing records.
